BackgroundCentral to ethically justified clinical trial design is the need for an informed consent process responsive to how potential subjects actually comprehend study participation, especially study goals, risks, and potential benefits. This will be particularly challenging when studying deep brain stimulation and whether it impedes symptom progression in Parkinson’s disease, since potential subjects will be Parkinson’s patients for whom deep brain stimulation will likely have therapeutic value in the future as their disease progresses.MethodAs part of an expanded informed consent process (...) for a pilot Phase I study of deep brain stimulation in early stage Parkinson’s disease, an ethics questionnaire composed of 13 open-ended questions was distributed to potential subjects. The questionnaire was designed to guide potential subjects in thinking about their potential participation.ResultsWhile the purpose of the study (safety and tolerability) was extensively presented during the informed consent process, in returned responses 70 percent focused on effectiveness and 91 percent included personal benefit as potential benefit from enrolling. However, 91 percent also indicated helping other Parkinson’s patients as motivation when considering whether or not to enroll.ConclusionsThis combination of responses highlights two issues to which investigators need to pay close attention in future trial designs: (1) how, and in what ways, informed consent processes reinforce potential subjects’ preconceived understandings of benefit, and (2) that potential subjects see themselves as part of a community of Parkinson’s sufferers with responsibilities extending beyond self-interest. More importantly, it invites speculation that a different paradigm for informed consent may be needed. (shrink)

Along with Charles Walraff's The Philosophy of Karl Jaspers, Schrag's work is the second book-length study of Jaspers' thought in as many years. As such it is very welcome, for Jaspers' philosophy has not yet been fully explored in English. And now that his three-volume Philosophie has been translated, we should see a great reawakening of interest in this distinguished German thinker. Schrag's book is an exposition of Jaspers' notion of the "Encompassing", that pivotal notion in his thought which (...) refers to Being as the primal and originary source of beings. The Encompassing can only be approached through its "modes," which Jaspers diagrams at the beginning of his lengthy treatise Von der Wahrheit. Schrag's book follows this diagram implicitly and its somewhat awkward title is based upon it. The Encompassing manifests itself in man on the one hand and in the non-human or objective on the other. Part One of Schrag's book is devoted to the "immanent" mode of the "Encompassing which we are," while Part Two discusses the Encompassing in man in its transcendent mode. The third and final part is devoted to the "Encompassing which Being itself is", which is a study of Jaspers' metaphysics. The reader will be especially interested in Schrag's Chapter Five, "Jaspers and Phenomenology," in which he discusses Jaspers' relationship to Husserl, who like Jaspers came to philosophy from psychology; and to Heidegger's "existential phenomenology." Almeder's book is a substantive contribution both to Peirce scholarship and to contemporary analytic epistemology. The great strength of The Philosophy of Charles S. Peirce is its engagement of Peirce's later thought with current issues in the foundation of knowledge and the philosophy of science. Almeder brings Peirce's ideas to bear on positions held by Quine, Sellars, Rescher, Hintikka, Scheffler, Popper, Feyerabend, and Russell and in so doing makes Peirce this group's contemporary and, in most cases, its philosophical better. (...) In addition, Almeder provides a rather sophisticated defense of epistemological realism, a defense broadly Peircean but supplemented considerably by Almeder's own friendly arguments. (shrink)

The first part of the paper sketches the rationale for the classical theistic thesis that, though God is not really related to the world, the world is really related to God. Part II delineates four sets of recent criticisms ofthis thesis: (a) an objection which assesses it as conflating transparent and opaque construals of intentional propositions, (b) a dilemma which regards it as undermining either free divine creativity or God’s knowledge of the contingent, (c) arguments which view its adherence to (...) an absolute divine immutability and independence as in conflict both with God’s knowledge of a changing creation and with human knowledge of God, and (d) two religiously inspired objections which contend that the thesis is contradictory to the biblical claim that God is personally related to the world. Part III develops the linguistic essence of the classical thesis through the syntactical insights of Sommers’ logic and defends it against the objections posed. (shrink)

1 Evidence-based policy requires researchers to provide the answers to ecological questions that are of interest to policy makers. To find out what those questions are in the UK, representatives from 28 organizations involved in policy, together with scientists from 10 academic institutions, were asked to generate a list of questions from their organizations. 2 During a 2-day workshop the initial list of 1003 questions generated from consulting at least 654 policy makers and academics was used as a basis for (...) generating a short list of 100 questions of significant policy relevance. Short-listing was decided on the basis of the preferences of the representatives from the policy-led organizations. 3 The areas covered included most major issues of environmental concern in the UK, including agriculture, marine fisheries, climate change, ecosystem function and land management. 4 The most striking outcome was the preference for general questions rather than narrow ones. The reason is that policy is driven by broad issues rather than specific ones. In contrast, scientists are frequently best equipped to answer specific questions. This means that it may be necessary to extract the underpinning specific question before researchers can proceed. 5 Synthesis and applications. Greater communication between policy makers and scientists is required in order to ensure that applied ecologists are dealing with issues in a way that can feed into policy. It is particularly important that applied ecologists emphasize the generic value of their work wherever possible. (shrink)

Returning to the text of his phrase-making Rede lecture Sir Charles Snow, surely an experienced explorer of the corridors of power, confesses to the awe of the sorcerer’s apprentice at the flood of comment, hostile and appreciative, near and far in origin, which his unpretentiously original thoughts upon the current rift in cultural communication conjured from the practitioners of literature and science, who broadly divide higher education in England today and whom he addressed at Cambridge in May 1959. The (...) widespread spontaneity of reaction has led him to conclude that his analysis touched a nerve, ‘something which people, all over the world, suspect is relevant to present actions’. His original lecture refused to relate itself to the political waging of ‘the cold war’ as ‘the prime absolute of our age’ and it castigated modernist literature for its neglect of the currently scientific stage of the industrial revolution, which is producing a new egalitarian society in our own time. Now he restates his major theme: the pressing need for a new education to promote common understanding between these two dominant cultures, so that the expanding social force of applied science be directed with the minimum danger and the maximum good for humanity. He does not minimise the danger of leaving a mystic status to the initiates of science. (shrink)

This is a welcome edition of the documents of a dramatic controversy of a century ago, which brought unexpected public glory upon a disappointed and brilliant convert to Catholicism. In 1864, a year of apparent neglect after nineteen years of Catholic life and a series of disheartening failures—as University Rector, translator of Scripture and editor of an intellectual journal—Newman was shocked by an incidental challenge to his integrity in a passing review by Charles Kingsley: “Truth, for its own sake, (...) has never been a virtue with the Roman clergy. Father Newman informs us that it need not, and on the whole ought not to be; that cunning is the weapon which Heaven has given to the saints wherewith to withstand the brute male force of the wicked world”. An unsatisfactory exchange of letters, published as a pamphlet by Newman led to Kingsley’s pamphlet reply, which in turn provoked the Apologia, or systematic History of My Religious Opinions. While a new critical edition of Hegel’s works is in progress, the only complete English version of his capital lectures upon the philosophy of history is the century-old translation by Sibree, made from the 1840 revised German edition of Hegel’s son, Charles. Hegelianism has since dramatically flourished, waned and has been distorted in a new materialist interpretation of history and political science. There is a large demand to-day for publishing the pure source of his doctrine on history. Dover Publications (...) render great service to the student in this elegant impression at a moderate price of a classic, to which it adds a succinct scholarly preface by Professor Friedrich. Here, we analyze the public health implications of recent legal developments — including privacy legislation, intergovernmental data exchange, and artificial intelligence governance — with a view toward the future of public health informatics and the potential of diverse data to inform public health actions and drive population health outcomes.

Analysis of sites of newly integrated DNA in cellular genomes is important to several fields, but methods for analyzing and visualizing these datasets are still under development. Here, we describe tools for data analysis and visualization that take as input integration site data from our INSPIIRED pipeline. Paired-end sequencing allows inference of the numbers of transduced cells as well as the distributions of integration sites in target genomes. We present interactive heatmaps that allow comparison of distributions of integration sites to (...) genomic features and that support numerous user-defined statistical tests. To summarize integration site data from human gene therapy samples, we developed a reproducible report format that catalogs sample population structure, longitudinal dynamics, and integration frequency near cancer-associated genes. We also introduce a novel summary statistic, the UC50, which provides a single number summarizing possible clonal expansion. Using these tools, we characterize ongoing longitudinal characterization of a patient from the first trial to treat severe combined immunodeficiency-X1, showing successful reconstitution for 15 years accompanied by persistence of a cell clone with an integration site near the cancer-associated gene CCND2. The American Psychological Association's (APA's) as well as other professional organizations' (e.g., American Psychiatric Association) removal of homosexuality as a mental disorder represented a paradigmatic shift in thinking about exual orientation. Since then, APA (2000) disseminated guidelines for working with lesbian, gay, and bisexual (LGB) clients, and a variety of scholars and researchers alike have advocated affirmative therapeutic interventions with LGB individuals. Despite these efforts, the controversy over treating individuals with LGB orientations using nonaffirmative techniques continues. In this discussion, the (...) limited evidence regarding the efficacy and effects of conversion therapy is surveyed, particularly in the context of empirically supported treatment criteria summarized by Division 12 (clinical psychology) of the APA. Authors then consider the resulting ethical considerations in performing conversion therapy and propose alternative uses of affirmative therapy on the basis of ethical standards defined by APA. Finally, options for treating LGB individuals who are coming to terms with their sexual orientations are discussed. (shrink)

Although epistasis is at the center of the Fisher-Wright debate, biologists not involved in the controversy are often unaware that there are actually two different formal definitions of epistasis. We compare concepts of genetic independence in the two theoretical traditions of evolutionary genetics, population genetics and quantitative genetics, and show how independence of gene action (represented by the multiplicative model of population genetics) can be different from the absence of gene interaction (represented by the linear additive model of quantitative genetics). (...) The two formulations converge with weak selection but not with strong selection or, for multiple loci, when the aggregated interaction terms are not negligible. As a result of the different formulations of gene interaction, the presence or absence of linkage disequilibrium,/D/, does not necessarily indicate the presence or absence of fitness epistasis. Indeed, linkage disequilibrium is generated in ‘additive’ models in quantitative genetics whenever two (or more) loci experience simultaneous selection. As a research strategy, it is often practical, for theoretical or experimental reasons, to minimize gene interaction by assuming independence of gene action in regard to fitness, or by assuming linear additive effects of multiple loci on a phenotype. However, minimizing the role of epistasis in theoretical investigations hinders our understanding of the origins of diversity and the evolution of complex phenotypes. (shrink)

Institutional review board delays may hinder the successful completion of federally funded research in the U.S. military. When this happens, time-sensitive, mission-relevant questions go unanswered. Research participants face unnecessary burdens and risks if delays squeeze recruitment timelines, resulting in inadequate sample sizes for definitive analyses. More broadly, military members are exposed to untested or undertested interventions, implemented by well-intentioned leaders who bypass the research process altogether. To illustrate, we offer two case examples. We posit that IRB delays often appear in (...) the service of managing institutional risk, rather than protecting research participants. Regulators may see more risk associated with moving quickly than risk related to delay, choosing to err on the side of bureaucracy.
